For Referring Physicians

At Hands-On Physical Therapy, our patient’s care is our top priority. We understand that by referring to us, you have placed your trust in our medical care and expect great outcomes.

For your convenience, you may download a physical therapy prescription and fax the script with your patient’s contact information.

Our office staff will contact the patient within a business day to schedule an appointment.

After the initial evaluation, you will receive an evaluation including a plan of care via fax for an update of our mutual patient’s status, and Progress notes (as needed) and a Discharge Summary upon completion of care.
